Can it bill Medicaid or the VA?

It does not submit claims. Record each program as a customer, invoice what you bill them, and apply the payments when they arrive.

Does it handle overtime for caregivers?

Yes. Enter overtime hours and rate on the paycheque. Whether a caregiver is due overtime depends on current federal, state or provincial rules; confirm with your accountant.
